A cash reward up to $2,500 is being offered to help track down a missing mother of four.

Sylvia June Galvan was last seen in Mt. Morris in October 2011.

She was just 30-years-old at the time.

Sylvia is 5’1”, and weighed around 150 pounds. She has brown hair, brown eyes, and moles on her right cheek and left side of her neck.

Her left ear has multiple piercings and her right ear has a single piercing. Her top teeth are also crowded and may stick out.

Sylvia also has several tattoos, including "Caldwell" on her upper arm, "Dennis and April forever" on her arm, "Howald" on her lower back and a rainbow with stars and a moon on her ankle.
